Acne Scars

Acne is a common problem not only in teens but all age group especially the complications that is the scars and pigmentation will stay forever. Prevention and appropriate treatment of ache is the most important method for acne scars and pigment management. At Forever Medspa & Wellness Center acne care and management are discussed in detail. Acne scars could be deep with less surface area which may look like wide deep pores or deep holes, these are ice pick scars. Box shaped deep scars are deep with steep wall and may look like box. Atrophic scars are thin stretchable depressed areas which when stretched will look like full correction. The visibility of the scars would increase if there is redness or pigmentation.

Following treatments or combinations will help in the appearance of the acne scars which otherwise is a permanent condition. Box scars and Icepick scars are initially treated and brought to the shallowness of the atrophic scars and then the treatment modality is similar. 

Box Scars/Ice Pick Scars

These are deep and firm which cannot be stretched or filled with filler, the method to treat theses is still increasing the amount of collagen in the depths of the scars. TCA cross which is 50% to 100% TCA when applied directly within the depth will create a controlled burn. The healing process will generate collagen right where it is needed. The layer eventually gets covered with skin epidermis. Sometimes repeat treatments are needed to fill in the scars all the way. Person with skin type 4 and above will need to be on skin lightening regimen prior and during this treatment. 

Atrophic Scars

Atrophic scars are thin stretchable depressed areas which when stretched will look like full correction. These scars are treated by creating a pull around the scar at the level of the dermis by generating collagen, simultaneously breaking the tightness of the scar. This is done by radio-frequency provided with special micro needles (RFMN). The microneedles provide the specific depth of the treatment along with breaking the tightness of the scars. RFMN can be ablative or nonablative, the former provides better results but has longer downtime and can be done only in skin type 1-3. The later hence need more treatments in range of 3-6 but could be done in all skin types with shorter downtime.

Other Treatments

Microneedling done deep every 4-6wks for 8-12 treatments could also produce visible improvement this could be added to above treatments in between. Microneedling produces collagen and reduce tightness mainly with mechanical damage without controlled thermal damage as off in RFMN.

Skin peels like VI Peels or Salicylic acid peels done every 6 wks can complement above treatment


Intense Pulse Light or IPL 

This is indicated when the scar base has redness, this redness comes from tiny capillaries that generate within the scar. These scars will have episodes when of increased or decreased visibility depending on how these capillaries react to the simulations around. IPL can target colors or reb, brown and black and hence can act on these capillaries. They get inflamed and either react with blanching or temporary increased visibility but eventually decreasing the redness in the base of the scar and reduced distinction form the surrounding skin. IPL doesn’t change the structure of the scar. 

Bellafill or Halluronic Acid Filler

Fillers help rolling or atrophic scars which can be improved with stretching and are sizable. Subcision that is undermining with a sharp needle is needed sometimes, before filling.
